Capture each full page in good light with readable text. Keep the name and date visible. Name files by date. Do not crop important information; agree any necessary translation with the coordinator.
CT, MRI and other imaging
The written report and original DICOM images provide different information. Ask the hospital which format is needed. Check access permissions and privacy before sharing large files; avoid posting medical files publicly.
Consent and missing records
Confirm which hospital will receive your records. Let the doctor or hospital identify missing information rather than ordering new tests yourself. Share a passport copy only when needed and through an agreed channel.
